WebLicensure by Examination or Late Renewal (credential expired more than 5 years): An applicant is eligible for licensure as a Massage Therapist or Bodywork Therapist if the applicant: 1. Completes application and pays appropriate fee(s) online via LicensE. 2. Submits a copy of current CPR/AED certificate from a DHS approved provider. Web(3) The applicant must have successfully passed one of the following examinations: (a) Federation of State Massage Therapy Boards massage and bodywork licensing examination; (b) National certification examination for therapeutic massage and bodywork; or (c) A board-approved examination.
